How to Complete Your POA

  1. 1.

Mark the box that matches your company type.

  2. 2.

Enter the Grantor's Employer Identification Number (EIN)/Federal Tax Identification Number in XX-XXXXXXX format. Individuals should enter their Social Security Number instead, formatted as XXX-XX-XXXX.

  3. 3.

Enter the Grantor's legal name exactly as registered with the IRS — it must match the full legal name tied to the number given in step 2.

  4. 4.

If the Grantor operates under a fictitious ("doing business as") name, list it here; otherwise leave this field blank.

  5. 5.

Enter the state where the Grantor conducts business.

  6. 6.

Provide the Grantor's physical address — street, city, state, and zip code. Foreign Grantors should also list their country. (A complete street address is required; P.O. boxes are not accepted.)

  7. 7.

Provide the email address where the Grantor receives electronic correspondence.

  8. 8.

Enter the state where the Grantor conducts business.

  9. 9.

Enter the Grantor's name again, matching step 3.

  10. 10.

Have an officer or authorized representative of the Grantor sign here.

    1. For a corporation, only the President, Vice President, Treasurer, or Corporate Secretary may sign — no substitutions.
    2. For a partnership, the signer must be a partner, and a copy of the partnership agreement is needed to confirm which partner(s) are authorized to sign on the partnership's behalf.
    3. For an LLC, signing authority is limited to a Partner, Managing Member, or an officer such as President, Vice President, Treasurer, or Corporate Secretary — no substitutions.
    4. For a non-resident entity, a non-resident certification is also required to confirm the signer's authority.
  11. 11.

    Print the signatory's name.

  12. 12.

Enter the signatory's title (e.g., President, Vice President, Treasurer — matching step 10).

  13. 13.

Enter the date the signatory signed.

A copy of the signatory's government-issued ID must be attached along with the POA.
